Have you cleared out your directory?
Delete everything but case.struct, do instgen, then run lstart.

I haven't seen that problem (and it may be unrelated to the structgen
problem). All I can tell you is what I already wrote in my previous email. I
only had any problems with the downloaded executables, so if that's what
you're running, then compile for your machine from the latest sources.
