Nice description!
Your item 5 is just a typo: it should be "x -h" instead of "x - h"
Issue 1 is probably not related to your errors.
For 2-4: what happens if you make a new clean directory with just the
struct file and then give (in this sequence) the following commands:
echo $WIENROOT
which x_lapw
which x
x nn
x sgroup
instgen
x lstart
